LBF Updates (January 8, 2013)On December 17, 2012, LBF 525 was updated to place the certificate of service below the debtor’s signature lines. LBF 5005 was updated to revise the list of items at the top, so that it now reads: Petition (including Exhibit D if applicable), Chapter 7 Debtor’s Statement of Intention, Declaration Concerning Debtor’s Schedules, Statement of Financial Affairs, Statement of Monthly Income, and Other.
